Facility Issue Reporting (FIR)

Facility Issue Reporting (FIR) is a tool that members of stake and ward councils, as well as the Stake Building Representative and Ward Building Representatives, use to report any issues that need to be repaired or addressed in a meetinghouse.

Procedures:

Funerals

Immediate Notification Required

As soon as it’s known that a funeral will occur, the following steps should be taken without delay:

  1. A member of the bishopric should immediately inform the Stake President.
  2. A member of the bishopric or the Ward Building Representative should immediately inform the Stake Building Representative (see Contact Information section above).
  3. The Stake Building Representative will reserve the building (rescheduling or relocating any activities already planned during the time of the funeral), and will notify the Mount Olympus Facilities Management Group via email to their office.
Snow Removal & Landscaping Issues

For non-emergency snow removal or landscaping issues that can be addressed within a few days to a week, please submit a Facility Issue Report (FIR). Including photos can be very helpful for the vendor.

In case of a snow removal or landscaping issue that must be addresses as soon as possible, please contact the 24-hour Building Emergency Hotline: (801) 273-3720, Ext. 1.

Technology Issues

The Stake Technology Specialist (see Contact Information section above) is the first point of contact for troubleshooting internet, networking, computer, printer, copier, sound system, and video system issues. As needed, the Stake Technology Specialist will make FIR submissions if an items needs to be repaired or replaced.

Keys & fobs
  • All keys and electronic fobs should remain with the calling to which they are assigned.
  • All Ward Building Representatives are responsible for maintaining a current list of keys and fobs for each calling, and conducting a yearly audit.
  • All requests for new or replacement keys or fobs should be submitted to the Stake Building Representative (see Contact Information section above). FIR submissions for keys or fobs will automatically be cancelled by the FM Group, and the member will be referred to the Stake Building Representative.

Instructions for Stake Building Representative:

For new or replacement keys or fobs, the Stake Building Representative submits a FIR request, which also serves as the Stake Building Representative’s approval. Ensure that all necessary information is included in the submission to expedite processing of the request:

  1. New fob request: Include ward name and calling.
  2. Replacement fob request: Include ward name, calling, old fob number, (or engraved number on back of fob, but fob number is preferable).
  3. Interior keys: When requesting an interior key, the service provider will need the information stamped on the back and front of the key. If this information is not available, specify the area the requested key should open and how many keys are needed. The service provider may ask for a copy of the needed key. Photos of the front and back of key can be uploaded with the FIR submission.
  4. Closet keys: When making FIR submission, include the closet number(s)/letter(s), the location inside the meetinghouse, and any information stamped on the key, if available.
Cleaning Supplies

Ward Building Representatives regularly monitor inventories of building cleaning supplies, consumable paper products, plastic garbage bags, ice melt, first aid kit supplies, and so forth. It may be helpful to keep an inventory list.

As supplies are diminished, Ward Building representatives submit reorder requests to the Stake Building Administrator (see Contact Information section above), who then orders the supplies from Church vendors. Please provide sufficient detail to ensure the correct product is ordered. A photograph of the product or its original contain is preferred.

The supplies are subsequently delivered directly to the Member Closet for the appropriate building within 1 to 5 business days.

Because supplies are replenished quickly, there is no need to maintain more than one month’s worth of supplies. For example, some supplies come in quantities sufficient to last for six months or more. It is only when they diminish to quantities to last for one month that they should be reordered.
